Loan Electronic Master Promissory Notes (eMPN)
Direct Stafford Loan electronic Master Promissory Note (eMPN)
If this is the first time you are borrowing a Direct Stafford Subsidized Loan or a Direct Stafford Unsubsidized Loan at DePaul, you must complete an
online electronic Master Promissory Note (eMPN).
Federal Direct Plus Loan (Parent Loan) electronic Master Promissory Note (eMPN)
You must complete an
online Plus Loan eMPN if this is the first time you are borrowing a Plus Loan at DePaul or if a different parent is now borrowing the Plus Loan.
Federal Graduate Student Plus Loan electronic Master Promissory Note (eMPN)
If this is the first time you are borrowing a Graduate Student Plus Loan, you must complete an
online electronic Master Promissory Note (eMPN).
Federal Perkins Loan Requirements
If you borrow a Federal Perkins Loan for the first time, you will need to visit the University Accounting Service (UAS)
Sign My Loan website to complete an online Electronic Master Promissory Note (EMPN) and Pre-Loan Counseling.
Each subsequent year that you borrow a Perkins Loan you must login to the
Sign My Loan website to affirm your new loan amounts and update your contact information prior to receiving your Perkins Loan.
Loan Entrance and Exit Counseling
Direct Stafford Loan Entrance Counseling
If you are a first-time Direct Stafford Loan borrower, you must complete the
Direct Stafford Loan Entrance Counseling Session. The purpose of the session is for you to learn your rights and responsibilities when borrowing student loans.
Federal Graduate Student Plus Loan Entrance Counseling
First time Graduate Student PLUS Loan borrowers must complete the
Entrance Counseling Online Session. Select the "Combination - Stafford and PLUS Loans" option and you will complete both requirements.
Direct Stafford Loan Exit Counseling
If you borrowed a Direct Stafford Loan and are graduating or leaving DePaul, you are required to complete the
Direct Loan Exit Counseling Session. During the exit counseling session, you will review your rights and responsibilities as a borrower.

Graduate Student Plus Loan Application Form
The Federal Direct Grad PLUS Loan Application Form supplement is necessary to process your Graduate Student PLUS Loan Application or your request for additional Graduate Student PLUS Loan funds.
If you would like to be considered for this loan, please visit
Campus Connection to request your loan online after you receive your award notice. Navigate to "View My Financial Aid" >> "Award Acceptance." The Federal Direct Grad PLUS Application link will be at the bottom of your Award Acceptance page.
Private Education Loan Applicant Self-Certification Form
This form is a new form that is required by the Department of Education. Private loan applicants must complete and submit this form to their private loan lender as part of the private loan application process. Borrowers will receive a copy of the form from their lender.
Complete the form and send it to your lender. This form does not need to be submitted to the Office of Financial Aid.
Change of Loan Request Form
If you would like to cancel, reduce or reinstate a financial aid award that you have been offered, you may do so by completing and submitting the Change of Loan Request Form to the Office of Financial Aid.

If your loan has been partially or entirely disbursed, please contact our office prior to submitting the Change of Loan Request Form. Once there has been a disbursement made on your loan, our office is limited in its ability to change that loan amount. In addition, if our office is able to change a loan amount that already has been disbursed to your student account, please keep in mind that we are removing loan funds from your student account, which may result in a balance due to the university.
