Loan Master Promissory Notes (MPN)

Federal Direct Loan electronic Master Promissory Note (MPN)

If this is the first time you are borrowing a Federal Direct Subsidized or Unsubsidized Loan at DePaul, you must complete an online electronic Federal Direct Loan Master Promissory Note (MPN).

Federal Direct Parent PLUS Loan electronic Master Promissory Note (MPN)

You must complete an online electronic PLUS Loan MPN if this is the first time you are borrowing a PLUS Loan at DePaul or if a different parent is now borrowing the PLUS Loan.

Federal Direct Grad PLUS Loan electronic Master Promissory Note (MPN)

If this is the first time you are borrowing a Federal Direct Grad PLUS Loan, you must complete an electronic Grad PLUS Master Promissory Note (MPN).

Federal Perkins Loan Requirements

If you borrow a Federal Perkins Loan for the first time, you will need to visit the University Accounting Service (UAS) Sign My Loan website to complete an online Electronic Master Promissory Note (EMPN) and Pre-Loan Counseling.

Each subsequent year that you borrow a Perkins Loan you must login to the Sign My Loan website? to affirm your new loan amounts and update your contact information prior to receiving your Perkins Loan.

Loan Entrance and Exit Counseling

Federal Direct Loan Entrance Counseling

If you are a first-time Federal Direct Loan borrower, you must complete the Federal Direct Loan Entrance Counseling Session. The purpose of the session is for you to learn your rights and responsibilities when borrowing student loans.

Federal Direct Loan Exit Counseling

If you borrowed a Federal Direct Loan and are graduating or leaving DePaul, you are required to complete the Federal Direct Loan Exit Counseling Session. During the exit counseling session, you will review your rights and responsibilities as a borrower.

Federal Direct Grad PLUS Loan

The online Federal Direct Grad PLUS Loan Application is necessary to process your Graduate Student PLUS Loan or your request for additional Graduate Student PLUS Loan funds.

If you would like to be considered for this loan, please visit Campus Connect to request your loan online after you receive your award notice. Navigate to "View My Financial Aid" >> "Award Acceptance." The Federal Direct Grad PLUS Application link will be at the bottom of your Award Acceptance page.

Request to Adjust Federal Loans Form

If you would like to cancel, reduce or adjust a student loan that has been offered as part of your financial aid award, you may do so by completing and submitting one of the Request to Adjust Federal Loans Form to the Office of Financial Aid.

If loan funds have already been applied to your student account, please contact our office prior to submitting a loan adjustment form. Once a loan has been disbursed to your student account, our office is limited in its ability to adjust your loan. In addition, if our office is able to change a loan amount that already has been disbursed to your student account, please keep in mind that we are removing loan funds from your student account, which may result in a balance due to the university.

Please notify us of any substantial change in income, expenses or circumstances that you wish to share with our office, by submitting the appropriate appeal form from the list below.

For a 2014-15 Income Related or Special Expense Special Circumstance Appeal submit this form.
For a 2014-15 Non-Income\Non-Expense Special Circumstance Appeal submit this form.

Be sure to submit all requested documentation to support your appeal.

Satisfactory Academic Progress (SAP) Appeal Form for Readmitted Students

If you are returning to DePaul after an absence and your were not making Satisfactory Academic Progress at the time you left, you will need to appeal for reinstatement for your student aid eligibility by submitting this form along with your SAP appeal letter and all supporting documentation.

Cost of Attendance Adjustment

If you wish to request consideration for an adjustment of your cost of attendance, please complete this form.

Be sure to submit all requested documentation to support your cost of attendance adjustment request.

Award Reinstatement Forms

If you wish to request reinstatement of an institutional scholarship, please complete this form and return it to our office.

If you wish to request reinstatement of other types of financial aid which were previously canceled, please complete this form and return it to our office. Please note that this form is not to be used for Satisfactory Academic Progress (SAP) appeals.
