DePaul University complies with the Illinois Department of Health, which requires
students to provide proof of immunization for Tetanus/Diphtheria/Pertussis,

Immunization Submittal Process
To submit your immunization records, be sure to have an electronic copy and then login online to Campus Connect > Student Center > Personal Information > Immunization Status (under other personal dropdown). You may also find it under Self Service > Admission > Next Steps > Immunization Status. Once you've submitted your immunization records, follow the same path to check your immunization status.
If you do not have a copy of your records, you may use the DePaul University Immunization Form to have your physician or health care provider complete and sign. Scan or take a photo of the completed form and login to the Campus Connect page and submit the form through the navigation path above.

- By law, DePaul University must prevent you from registering in subsequent terms if you've failed to submit documentation verifying that you meet the immunization requirements.
- All records must be in English or be accompanied by a certified translation.
- DePaul does not re-release immunization information. A copy of your immunization status page can be printed through Campus Connect.
