circumstances by submitting the
appropriate appeal form. Be sure to submit all requested documentation to support your appeal.

Satisfactory Academic Progress Appeal Form for Readmitted Students
If you are returning to DePaul after an absence and you were not making Satisfactory Academic Progress at the time you left, you will need to appeal for reinstatement for your student aid eligibility by submitting an online form along with your SAP appeal letter and all supporting documentation. Please use your Campus Connect login credentials to access the online form.
Cost of Attendance Adjustment
If you wish to request consideration for an adjustment of your cost of attendance, please complete the Cost of Attendance Adjustment Request Form. Be sure to submit all requested documentation to support your cost of attendance adjustment request.
Award Reinstatement Forms
If you wish to request reinstatement of an institutional scholarship, please submit a Scholarship Reinstatement Appeal. Log in with your Campus Connect username and password.
If you wish to request reinstatement of a cancelled financial aid package or declined Federal Direct Student Loans, please complete the online Request for Award Reinstatement. You should use your Campus Connect login credentials to access the form. This form cannot be used for Satisfactory Academic Progress (SAP) appeals.
