Special Circumstance Appeal Forms

Notify us of any substantial change in income, expenses or circumstances by submitting the appropriate appeal form and all requested documentation to support your appeal.

For academic year 2020–2021 (Fall 2020, Winter 2021, Spring 2021 and Summer 2021):

Satisfactory Academic Progress Appeal Forms

  • All students with Campus Connect credentials go to Campus Connect > Financial Aid tile > View My FA Academic Progress > Online Appeal Form
  • SAP Appeal for Readmitted students: Students readmitted to DePaul after an absence of three consecutive terms and do not have access to the Campus Connect online appeal form should email OFA_SAP@depaul.edu.

Cost of Attendance Adjustment

Request an adjustment of your cost of attendance by submitting the appropriate appeal form and all requested documentation to support your request.

Award Reinstatement Forms

  • Scholarship Reinstatement Appeal: to request reinstatement of an undergraduate academic scholarship (use your Campus Connect login credentials).  If you have questions regarding a graduate or professional academic scholarship, please email ofascholarships@depaul.edu
  • Request for Award Reinstatement: to request reinstatement of a cancelled financial package or declined Federal Direct Student Loans (use your Campus Connect login credentials). This form cannot be used for Satisfactory Academic Progress (SAP) appeals.
