Special Circumstance Appeal Forms

Notify us of any substantial change in income, expenses or circumstances by submitting the appropriate appeal form and all requested documentation to support your appeal.

For academic year 2021–2022 (Fall 2021, Winter 2022, Spring 2022 and Summer 2022):

For academic year 2020–2021 (Fall 2020, Winter 2021, Spring 2021 and Summer 2021):

Satisfactory Academic Progress Appeal Forms

  • Current Students: By the deadline established in this SAP FAQ, complete a SAP appeal in Campus Connect. Meet with your advisor to complete the online SAP advisor support form in Campus Connect.
  • Readmitted Students: Only readmitted students, who were not meeting SAP prior to their absence, can appeal by submitting an online Satisfactory Academic Progress Appeal for Readmitted Students Form (use your Campus Connect username and password to login to this form).

Cost of Attendance Adjustment

Request an adjustment of your cost of attendance by submitting the appeal form and all requested documentation to support your request.

For academic year 2021–2022 (Fall 2021, Winter 2022, Spring 2022 and Summer 2022):

Financial Aid Reinstatement Forms

Scholarship Reinstatement Appeal: To request reinstatement of an undergraduate academic scholarship, complete the Scholarship Reinstatement Appeal in Scholarship Connect (use your Campus Connect username and password to login). You will be required to submit a General Application and sign a Consent Statement before completing the appeal, if you have not done so for the academic year already. Financial Aid Reinstatments:

For academic year 2021–2022 (Fall 2021, Winter 2022, Spring 2022 and Summer 2022):

  • Request for 2021-2022 Financial Aid Reinstatement: to request reinstatement of cancelled 2021-2022 financial aid or declined Federal Direct Student Loans beginning with the Fall 2021 term (use your BlueSky login for this form). This form cannot be used to apply for summer financial aid or for Satisfactory Academic Progress (SAP) appeals.

For academic year 2020–2021 (Fall 2020, Winter 2021, Spring 2021 and Summer 2021):

  • Request for 2020-2021 Financial Aid Reinstatement: to request reinstatement of cancelled 2020-2021 financial aid or declined Federal Direct Student Loans beginning with the Fall 2020 term (use your Campus Connect username and password to login to this form). This form cannot be used to apply for summer aid or for Satisfactory Academic Progress (SAP) appeals.
